Registration

2012-2013 Attendance Requirements

To attend the Los Gatos Union School District, a student must be:

  • at least five (5) years of age on or before December 2nd of that academic calendar year
  • reside on a full-time permanent basis (at least four days and four nights) within the boundaries of the District.

 

If the student meets these requirements, the parent/guardian may register the student.

Kindergarten and Transitional Kindergarten (TK) registration packets for the 2012-13 school year will be available beginning Wednesday, February 1, 2012 at the following schools in the Los Gatos Union School District:

  • Blossom Hill Elementary School
  • Daves Avenue Elementary School
  • Lexington Elementary School
  • Louise Van Meter Elementary School

Children who will be 5 years old on or before December 2, 2012 will be eligible to register for placement in August 2012. Birth certificate, proof of residency, and immunization records will be required before your child enters school in the fall. Only completed packets as requested in the enrollment checklist will be accepted starting Monday, February 13, 2012 at each of the elementary school sites.

2012-2013 Registration Requirements

The following ORIGINAL documents must be provided to the school office to begin the registration process. You may download and print the forms or pick up a packet at the school office.

  1. New Student Registration - Form
  2. Proof of Residence For Enrollment - Form
  3. Residency Declaration - Form
  4. Notarized Residency - Families in shared housing ONLY - Form
  5. Health Information - Form
  6. Child's Birth Certificate (original & a photocopy)
  7. Photocopy of Shared Custody Agreement (if applicable)
  8. Current Immunization Record (original & a photocopy)
  9. Oral Health Assessment/Waiver (K-1st ONLY) - Form
  10. Immunization Requirements & TB Test Form K-5th or 6th-8th
  11. Buzz Book-ONLY if not completing the online registration- Form
  12. Updated immunization (original & a photocopy)
  13. Health Examination for School Entry - 1st grade ONLY - Form

After the required documents are processed, you will receive a Snapcode that will allow you to complete the online registration process.

Campus Visitations

Campus visitation for Kindergarten and Transitional Kindergarten students will be held at each school site on:

  • Date: May 2, 2012
  • Time: 1:00 - 2:00 p.m.
  • Location: At each elementary school site
