Student Registration

Beginning February 2, 2009, Kindergarten and Grades 1-8 registration packets can be picked up at the following schools in the Los Gatos Union School District. We have provided the Registration form below but you must visit the school office to pick up the complete packet--Only complete packets will be accepted beginning February 11, 2009.

Those children who turn 5 years old by December 2, 2009 may register for Kindergarten.

Campus visitation dates, for parents of Kindergarten and other students new to the school district, are listed below.

Requirements for Registration:

• Birth Certificate
• All current immunization

Proofs of Residency:

• 3 bills (includes; 2 utility and one other bill)
  
• DMV Registration, OR driver's license, OR DMV ID with current address in LGUSD school area.
• W2 OR 1099 form with current address in LGUSD school area (financial information blacked out).
• County tax bill, close of escrow papers, deeds OR Official Rental Agreement
  

Not Required at Time of Registration:

• Report of Health Examination (to be filled out by physician at child's 5 year checkup and turned in asap or before school starts. Grades K and 1 only)
  
• TB Form (small, goldenrod; turned in asap or before school starts)
  
• Oral Health Assessment OR Waiver Request Form (white, Grades K and 1 only)
